I have seen this distinctive handle back a few times before, all on tablespoons, but not by makers I know.

There is nothing else peculiar about the piece. The maker's mark is a somewhat crude D. F. JOHNSON

Does anyone know if this is a regional variation?

No drop. Normal fiddle form with fairly wide flared shoulders. Definitely American in origin.
